For your initial weight loss appointment you meet with a medical provider to choose a medically supervised weight loss program that will work best for you.  To decide this we use a thorough Health Intake form that includes medical history and lifestyle habits.

 

Once a program is chosen you will meet with a medical provider weekly to help keep you on track, take measurements, review, revise and educate.

Initial Cost: $475 Includes medical exam, first month's membership with B12/
                     Lypo-M injection at each visit and 30 day Medifast Variety
                     Starter Kit

 

 Monthly Membership: $95/month plus cost of Medifast Meals
                    Monthly Membership includes 4 office visits with B12/Lypo-M
                     injection at each visit.  Office visits should be done once a week

 

Additional Weight Loss Supplies:

*SB MEDICAL WEIGHT LOSS does not bill insurance companies.  However we provide you with the diagnosis billing codes and invoice for you to submit to your insurance carrier.  Prescriptions may also be covered.

 

Healthcare Spending Accounts or Flexible Spending Accounts may be used.  Please check with your insurance professional/administrator.
